This survey is about more than numbers—it’s about capturing the reality of what living with herpes looks like so we can show public health professionals, therapists, and allies how to better help us and educate the general public more accurately from our experiences. Taking this 20-30 minute anonymous survey will greatly help us create more resources and advocacy that reflect our community’s needs.

Who’s it for? Anyone who has had a physical outbreak or clinical herpes diagnosis.
How long does it take? 20-30 minutes, and it needs to be done in one sitting.
Is it anonymous? Absolutely. No identifying information is collected, and you can only submit once.

Your experience with physical symptoms, outbreaks, and medication.
Navigating relationships and sexual health including discussions around disclosure.
