SPFPP intake forms are used solely for Courtney to have a little information about you to maximize your time together on the support calls. Surveys are always anonymous and we only have and use your responses for our advocacy efforts. These do not require your name or any identifying information that can link to you unless you request to be contacted.

MISCELLANEOUS

Any correspondence is considered confidential unless specifically discussed and agreed upon (such as a public podcast guest). Your private information remains between us (SPFPP and you) unless you voluntarily relinquish that anonymity to connect with someone upon request.
